Fill this out before your first class and you will not spend the first half hour of range time filling out paper on a clipboard. Only your name and email are required — everything else can be completed later.

Your information goes directly into your Alpha One student file. We do not sell, share, or market it — see section 09 below. If you would rather do this in person, that is fine too; just tell us when you arrive.

06 Training records & legal importance

Firearms and security training records may be legally significant in the State of Florida.
Courts, employers, and licensing authorities may consider documented training when evaluating competence, compliance, or conduct.
Alpha One Firearms Training maintains training records as part of its duty to train and duty to document.
You are responsible for maintaining your own copies of any training documentation provided to you.

07 Consent to communication

By enrolling in training, you consent to allow Alpha One Firearms Training and its authorized staff to contact you for training-related purposes.
This includes course scheduling, updates, requalification reminders, licensing communications, documentation requests, and record issuance.
Methods of contact may include email, phone calls, and SMS/text messages.
This communication is transactional and training-related, not unsolicited marketing.
You may opt out of non-essential communications at any time by notifying Alpha One in writing.

08 Licensing & compliance acknowledgment

Licensing information is requested solely to verify eligibility, compliance, training status, or reporting requirements under Florida law.
You are responsible for ensuring all licensing information provided is truthful, accurate, and current.
Alpha One may retain licensing-related information only as necessary for training, regulatory, or recordkeeping purposes.

09 Privacy & data use

Student information is not sold, shared, or marketed to third parties.
Information is used only for training, administrative, regulatory, or legal recordkeeping purposes.
We do not collect medical records, financial account numbers, or other protected personal data.
Information may be disclosed only when required by law, regulation, court order, or licensing authority.

10 No legal advice

Alpha One Firearms Training provides training and education only. Nothing in this document constitutes legal advice.
